Accidents are unplanned and unintentional events that result in harm or loss to personnel, property, production, or nearly anything that has some inherent value. Accidents occur every day everywhere. Accidents are rarely simple and almost never result from a single cause. Most accidents involve multiple, interrelated causal factors. Investigating incidents is important to learn what went wrong so repeat occurrences can be prevented. Understanding how to prevent or control accidents requires an understanding of the sequence of events leading to an accident in order to identify and implement countermeasures that contain risks. It is for these reasons that the GHANA INSTITUTE OF SAFETY AND ENVIRONMENTAL PROFESSIONALS (GHISEP), at the beginning month of the second half of the year, invited one of the best safety professionals and a founding member of GhISEP with in-depth knowledge in Accident Investigation, in the person of Samuel K.
